INSTRUCTIONS FOR IN-STATE OR LOCAL CLIENTS:
If you are a new client, please go here to download the clinical appraisal form.
Print out and complete all areas of form and bring on day of your appointment.
Appointments will be one hour long. Diet, nutrition, lifestyle factors will be covered. Call 678-663-4411 to schedule an appointment.
* It takes about 1-2 weeks to receive test results.
* If we are not in your local area, and or you are out-of-state, we can still help you, call us for more information and you can send us your Hair and Nails through the mail. Read the out-of-state instructions for collecting Samples.
INSTRUCTIONS FOR OUT-OF-STATE CLIENTS:
If you are a new client that is out-of-state or not local to our office area follow the instructions provided below.
Download clinical appraisal form, print and fill out.
Next, You will then take samples of your Hair and Nails. You will also need two, lunch-sized zip-lock bags to insert Hair, Nails, separately.
1. Cut out a teaspoon amount of hair. It has to be hair from the back of head close to the scalp as possible. Insert hair into zip-lock bag and label it "Hair." Also write birthdate on the bag.
2. Cut toenails or fingernails, about 6-8 clippings. insert clippings into a new zip-lock bag and label it "Nails." Also write birthdate on the bag.
After you have taken samples and filled out Clinical Appraisal form, then you will mail out the samples, clinical form, and check payment ($350) in an envelope of choice to the address:
